TVHC’s CQOW Team Showcases QI Model at OCHIN Learning Forum

In early May, members of TVHC’s Clinical Quality & Operations Workgroup (CQOW) had the opportunity to attend and present at the 2026 OCHIN Learning Forum, joining health centers from across the country to share ideas, learn from peers, and advance quality improvement work. Medical Director of Population Health and Quality, Family Medicine Dr. Diana Mokaya, Director of Clinic Operations Aldo Frausto, and Quality Analyst Hector Sanchez led a presentation highlighting how CQOW’s structure is strengthening our approach to clinical quality and operations.

Their presentation focused on how TVHC has built a coordinated, cross-functional model for quality improvement by bringing together clinical, operational, and data teams into shared workgroups.  This approach allows teams to move more quickly from idea to implementation while using data to continuously guide decision‑making.

CQOW’s structure was also highlighted as a key driver of progress across the organization. Through dedicated workgroups aligned with priority areas such as blood pressure control, cancer screening, initial health assessments, well‑child visits, and youth dental access, teams can focus on clearly defined goals while collaborating across departments.  This model has helped improve engagementstrengthen workflows, and support measurable improvements in performance.

Sharing this work at the OCHIN Learning Forum not only highlights the progress happening at TVHC but also contributes to a broader effort to strengthen care across the community health center network. The CQOW model was presented as a practical and replicable approach that other organizations can adapt to improve their own quality initiatives, particularly in areas tied to regulatory requirements, value‑based care, and population health.  At the same time, our team was able to learn from peers nationwide, bringing back new ideas and insights to help elevate ongoing projects at TVHC.  

Outside of their presentation, the team participated in additional sessions focused on clinical care, operations, and data strategy, gathering insights that will help inform future improvements across our organization. These learnings will continue to support CQOW’s work as it evolves and expands in the coming year.

Thank you to Dr. Mokaya, Aldo, and Hector for their leadership and for representing TVHC on a national stage! Their work reflects the strength of CQOW and the collective efforts across our teams to continuously improve care for our patients and communities.
