Building Bridges: Innovation in Action with Behavioral Health
TVHC’s Behavioral Health Department was honored to present “Building Bridges: Innovative Integrated Care for Latino Immigrant Communities” at the California Primary Care Association Conference. Director of Behavioral Health Services Dr. Jessica Apfel and CyB Supervisor / Mental Health Specialist Monica Zúñiga shared how TVHC is developing sustainable, community-centered approaches to address some of today’s most pressing healthcare challenges.
As immigrant communities face increasing barriers to healthcare access, heightened stress, and growing health disparities, our teams have responded by strengthening partnerships between behavioral health, primary care, health education, and community support services. Rather than treating physical and emotional health separately, our integrated care model brings them together to improve outcomes, increase engagement, and foster healing.
Integrated care models such as Familia Adelante and the Wellness Support Group have demonstrated that culturally responsive, prevention-focused services can make a meaningful difference. Participants report reduced anxiety and isolation, stronger social connections, improved self-management of chronic health conditions, and a renewed sense of hope and belonging. Many describe these programs as life-changing opportunities to heal, connect, and thrive.
At a time when prevention and community-based services face significant funding challenges, our experience shows that integrated care is not only effective, but also essential. By leveraging collaboration, cultural strengths, and existing resources, community health centers can continue advancing health equity and supporting the resilience of the communities we serve.
