Under Pressure: Strengthening Blood Pressure Control
Controlling high blood pressure is a key driver of long-term health outcomes and a major focus of HEDIS performance. Through the Under Pressure initiative, CQOW works to strengthen clinical workflows, staff training, and technology tools to better support patients with hypertension.
The initiative focuses on refining clinical pathways using Epic and OCHIN tools, including Our Practice Advisories (OPAs) that alert providers when patients have repeated elevated blood pressure readings. These alerts help prompt timely follow-up and clinical decision-making.
CQOW also emphasizes Medical Assistant training on repeat blood pressure measurements when initial readings are elevated. This practice is tracked through the Cardiovascular Risk Reduction Epic dashboard via the “BP Measurement” dashboard component, allowing teams to monitor consistency and impact.
Additionally, our team was able to elevate this prebuilt component through increased capabilities within Epic, making our own custom “SA183 BP Recheck by Site.” With these innovative tools, TVHC’s Union City clinic has more than tripled its rates of blood pressure rechecks since June 2025.
In collaboration with Clinical Pharmacy, CQOW is also supporting medication management for patients enrolled in CareSignal remote blood pressure monitoring. This integrated approach helps ensure patients receive ongoing support beyond in-clinic visits, improving control and reducing cardiovascular risk.
