TriZetto Breach: FAQ For Patients

As a patient of Tiburcio Vasquez Health Center, you may have recently received a letter notifying you of a recent breach to TriZetto, a third-party company that works with our electronic medical record system (OCHIN).

  1. I just got a letter saying my identity may have been stolen. What happened?
    TriZetto, a national organization that works with health care companies across America, noticed that someone had gotten into their databases without permission.
  2. Was my information stolen?
    We do not know for sure, right now we do not know if anything was stolen at all. There is the possibility, which is why we are letting you know.
  3. If so, what was stolen?
    We do not know for sure if anything at all was stolen. The information that was at risk includes: Names, Addresses, Dates of Birth, Medical Record #s, Health Plan/Patient ID#s, and Phone Numbers.
  4. What should I do now?
    You should receive a letter soon from a company called Kroll. This is the company that TriZetto has hired to help members whose information may have been stolen. Follow the instructions from Kroll as soon as you receive the letter.
