Hypertension Group Medical Visit Program

Program Description

Tiburcio Vasquez Health Center invites you to a new type of group medical visit that supports patients with Hypertension, or high blood pressure (BP), in reaching their health goals. 

There are 2 components to each session: 

  • Group Visit: learn with a health coach & peers how to have healthy blood pressure.  
  • Doctor Visit: brief private visit with a doctor about your blood pressure status. 

Included In This Program

Patients receive the following (free of charge): 

  • A Bluetooth-enabled home blood pressure cuff 
  • Enrollment in CareSignal, our text message-based blood pressure monitoring service 
  • Assistance with your medical record (MyChart) 
  • Between visit communication, care coordination and support towards reaching your health goals
  • Educational resources including recipes, stress/pain/weight management techniques and medication information 

Schedule 

This group will meet virtually, twice a month for 6 months, for a total of 12 sessions. Each session lasts 90 minutes and is conducted using the Zoom web-conferencing application. You may use your phone or a computer for this.  

Spanish-language Sessions 

Spanish sessions will be offered Tuesday mornings, 9:30 AM – 11:00 AM. 

English-language Sessions

English sessions will be offered Friday afternoons2:30 PM – 4:00 PM.

CareSignal 

Eligible patients will receive a phone call or text message to enroll. You will select two days per week to receive your text or automated phone call. It only takes a few minutes each week to respond to TVHC CareSignal messages. Respond to at least one message each month and be entered to win a $50 Walmart gift card! 

MyChart 

MyChart is the online portal for your health information.
