Thank you to all our grant-makers and donors that helped us start 2022 off with a bang!
Below is the funding we received in the month of January.
Through a partnership with Alameda County Public Health Department, TVHC was awarded a two-year contract to be the lead agency in a joint venture coalition with partners Hayward Area Recreation and Park District (HARD, Eden United Church of Christ, Ruby’s Place, and Kidango. Funding will allow TVHC and our coalition partners to expand COVID-19 vaccine access and support community outreach and resilience activities to community members in 94544 (South Hayward), 94541 (Ashland-Cherryland), 94578 (Ashland-Cherryland (Ashland), 94580 (Ashland-Cherryland (San Lorenzo), and 94538 (Fremont). Tune in later to hear more updates about the work TVHC and our amazing partners are doing in this coalition.
Thank you to long-time supporter, Sobrato Philanthropies, for providing general funding support through their Essential Human Services grant. This funding will be used to support technology and health equipment and supplies.
Thank you to CHCN and Anthem BlueCross for supporting TVHC’s Blood Pressure Care Pathway Program with funding that will allow us to expand the use of Self-Measured Blood Pressure (SMBP) monitoring for Anthem Medi-Cal Managed Care members. Funds will support the technology and equipment required to continue ensuring TVHC patients manage and improve their blood pressure.
